Control your Blood Pressure and Cholesterol

Know Your Numbers

  • Blood Pressure and Cholesterol Poster: for provider exam rooms. Poster includes sticky nores for provider to record patients blood pressure and cholesterol values, which the patient can take home as a reminder and motivator to regularly monitor and manage their numbers.
  • Poster Sticky Notes: More sticky notes for when you run out.
  • Dry Erase Board: This write on board and marker can be used by patients/clients and includes space to record blood pressure and cholesterol values, a "Things to Do" list, and emergency telephone numbers.

  • HeartSafe PowerPoint
    Presentations to assist HeartSafe designees and partners in promoting their HeartSafe designation status, and educating community members about the signs and symptoms of sudden cardiac arrest, heart attack and stroke, and the need to call 911 immediately. These presentations also include cardiovascular disease statistics, risk factors and how to reduce them.
  • HeartSafe Community Resolution
    A tool to help designees and partners promote and gather support from decision-makers and community members, including maintenance and enhancement of their community’s cardiovascular chain of survival.
  • Maine Survivor Stories
    These are true stories of Maine people who have survived heart attack, stroke and/or sudden cardiac arrest. Survivors and family members share their experiences and what they learned over the course of recognizing symptoms and receiving life-saving treatment for their cardiovascular events.

    • 5-Minute Heart Attack Handout: This resource briefly highlights heart attack symptoms, the need to call 911 immediately and types of treatment for heart attack.
    • Signs of Heart Attack Magnet: A great tool to help people remember heart attack symptoms and the importance of calling 911.
    • Audience Handouts15 minute | 30 minute: These handouts can be used by audience members to follow along with 15 and 30-minute presentations/discussions. The handouts include slides from each of the Community Presentations, as well as space for audience members to jot down notes and questions.

    • In a Heartbeat Project Outline: This outline describes the Maine Quality Forum’s state-wide initiative to improve heart attack systems of care.
    • Community Outreach Project Outline: This outline describes community engagement initiatives to increase Maine’s knowledge of heart attack symptoms and the importance of calling 911.
    • Planning Tip sheet: This resource assists trainers in planning successful outreach efforts. It includes a planning timeline and tips to engage your audience.

      A training presentation that assists state-wide partners in training other groups within their communities to deliver consistent messages around heart attack (program background, CVD statistics, partnership roles, training resources, audience resources, etc.)
    • Community Presentations to Increase Heart Attack Awareness
      15 minute PDF | 30 minute PDF | 15 minute Powerpoint | 30 minute Powerpoint: These presentations can be utilized as a PowerPoint, or in a less formal discussion format. There are 15 and 30-minute versions to accommodate various times and audiences. The primary focus of the presentations is to teach community audiences the symptoms of heart attack and the importance of calling 911. They also cover heart attack risk factors and ideas to reduce risk.
    • Trainers’ Guides15 minute | 30 minute: These Guides are designed to help trainers deliver consistent heart attack awareness information. There are Guides for both 15 and 30-minute presentations/discussions, which include slides from each of the Community Presentations, as well as notes pages to assist trainers with content and ideas for audience participation.
    • Audience Survey Template: For those wishing to evaluate the outcome of your community presentations, we have provided a survey that will help measure pre/post education knowledge among your audience participants.
